Accommodations for Candidates with Disabilities

If you need special accommodations because of a disability, fax/mail a current letter from a licensed physician, school official, licensed psychologist, or other appropriate authority on his or her letterhead (including title, address, and telephone number) describing the nature of the functional limitation as it applies to taking a multiple-choice standardized test and the special accommodations you will need for testing.
